Illness has an uncanny way of interrupting our plans. At Milestones and Transitions, we will help you make new plans. We will spend time with you to get to know what is important to you, and we will work with your family and with your medical team to learn about your illness and identify your needs. We will call on your personal, familial and community resources as appropriate to meet your needs. Based on our understanding of the impact of your illness on your overall situation, our professional assessment will make recommendations which are intended to bring you the peace and confidence which we now know to be so essential to maximizing the benefits of your treatment.
